Patient Partnership Statement

1.      I will order my medicines before I run out and only order what I need.

2.      I will let my practice/pharmacist know if I stop taking a medicine or have a problem.

3.      I agree to undertake any essential health checks (blood tests/monitoring) in advance of my medication review.

4.      I understand my prescription might not be reissued until a review/required tests are complete.

5.      I will return any unused medicines to a pharmacy for safe disposal.
